whatever the problem was last time it was clearly only solved through better nutrition. if you can't get better nutrition into him during the winter months then no it isn't fair to put him through this. his face is sore because hes desperate to eat and can't.....

My Vets usually suggest any relevant (and expensive) treatment before suggesting having an animal PTS. If the incisors are the problem I'm surprised your vet hasn't suggested removal, or even rasping them down - its pretty common practice in horses.  I really don't think the 'wait and see if he picks up' option will work, chances are he'll just starve to death.
