One note though Felicity, once you've collated the responses, it would be really interesting if you could post a wee summary of what you've found.  Also, it would be really interesting to know what the health implications behind certain questions actually were. For example, yes, wild animals do have access to our hens' water supply, as it's a stream that runs through their area. The question is, what are the likely implications of this, and what should I be looking out for?

On a similar note, we have a bit of a rat problem, they are all around us!  But they only ever go for hens that have died & are still warm, or very very young chicks. I struggled to explain this on the questionnaire
Do rats need their own question?!
