Paperwork needed for abbatoir

I am a first time pig owner taking two pigs to abbatoir tomorrow morning and am confused about paperwork needed.  Have just spoken to Animal Health department who have said just AML2 form needed plus slap mark although have been told by somebody else that there is a DEFRA form to download also.  Can anyone please definitively tell me what I need??!

I think you mean the Food Chain Information Form? (FCI)

Your AML2 form, the FCI form from BPEX and make sure the pigs are identified as your abattoir wants them.  Some require metal ear tags, some a slap mark...best to check with your particular abattoir to see what they need.
